TRUST REPLACEMENT and AMENDMENT FORMS
It is recommended that you download an "amendment form" along with the trust replacement pages. There are two different amendment forms. One simply changes any form in the trust. The other will add a form page, change a form page or revoke any gift or page. Complete the proper amendment form and attach the new replacement page to it. We recommend placing the original amendment form and page with the original documents in your safe deposit box. A copy of the amendment should be kept in your 3 ring binder and placed in front of the trust.

WILL REPLACEMENT and CODICIL FORMS
In order to amend a Will it is necessary to prepare and sign a form called a "codicil." This two page codicil form is signed and witnessed like the original will. Attached to the codicil is the desired replacement page. Download a codicil along with the single or married person's will form. A married couple must each use a codicil for each of their own wills. The original forms should be kept in the safety deposit box along with the original documents. A copy of the codicil and amended page should be kept in the 3 ring binder in the Will section.
